Portfolio of Model Programs 2011-2022
For more than a decade, the Upstream Investments Portfolio helped advance the adoption of evidence-informed programs across Sonoma County. As evidence-informed practices became more widely implemented and priorities evolved to focus on collective impact and Anti-Racist Results-Based Accountability (ARBA), the Portfolio Review Committee was dissolved in 2022 and the application process transitioned to an administrative review. With no new applications received since that transition, the Portfolio application process has now concluded, and no new applications are being accepted.
The list of Portfolio programs that completed the application process and were approved prior to 2022 will continue to be highlighted as a legacy resource on the Upstream website – link here. The Upstream team is grateful for the programs that have and continue to contribute to Sonoma County's prevention-focused efforts.
The practices accepted to the Upstream Investments Portfolio are focused on strengthening individuals and the community to prevent social problems before they start. The program design must be based on data and research that shows the techniques used to be effective.
- Evidence-Based Practices
- Evidence-Informed (Promising) Practices
Both must demonstrate that they have the potential to have a positive impact on the community by preventing problems before they start.
